TNR Surgery Permission Form Thank you for trapping stray cats in San Saba County and allowing us to spay/neuter them. Trap-Neuter-Return (TNR) programs have been proven to be both humane and effective at controlling feral cat population growth. You must check each box below to agree to each condition and sign the form. Consent(Required) 1. I give permission for this stray cat(s) to get spayed or neutered at low-cost clinic chosen by San Saba County Friends of Animals. I understand that San Saba County Friends of Animals will incur all costs associated with this procedure; although, all donations are greatly appreciated.(Required)Consent(Required) 2. I understand that the cat(s) must remain in a live trap due to temperament and to have a safe surgery performed.(Required)Consent(Required) 3. I will return these cats to the same location where I trapped them. I understand this is in the best interest of the cat and that fixing all the cats in one colony will help stabilize the colony and help to prevent any new cats from moving in to the area. I agree to provide adequate food, water, and shelter (barn, storage shed etc.) to the cats after their return.(Required)Consent(Required) 4. I also give San Saba County Friends of Animals permission to make all decisions regarding the cat’s health. This includes but is not limited to rabies, flea/lice medication, antibiotic shot, and/or euthanasia if determined medically necessary by the attending veterinarian.(Required)Consent(Required) 5. I understand that the cat will receive an ear tip, no exceptions. (Ear-tipping is a visual way to identify outdoor community cats that have been spayed or neutered. It is a standard practice used nationwide in the US.)(Required)Consent(Required) 6. I understand that if the cat is pregnant at the time of surgery, the pregnancy will be terminated. I will do my best to keep any cats in heat away from intact males for at least 7 days after the spay surgery as they will finish out their heat cycle and there is a risk of infection if breeding is attempted.(Required)Consent(Required) 8. I understand that the risk of injury or death, although extremely low, is always present when performing a spay/neuter, because there is some risk associated with this procedure as well as the use of anesthesia.
